Seminar Objectives:

Program 1 - Upon completion of this seminar, participants will be able to:

1. Identify the population of patients who would be at risk of stroke with atrial fibrillation or deep vein thrombosis
2. Describe the benefits of oral anticoagulants for stroke prevention in atrial fibrillation and deep vein thrombosis
3. Discuss the benefits and risks of oral anticoagulant therapy for reducing the risk of stroke in atrial fibrillation patients and deep   vein thrombosis
4. List counselling points to educate patients on adverse events associated with anticoagulants

Program 2 - Upon completion of this seminar, participants will be able to:

•Explain the key components of the medication therapy management (MTM) 
•Define the 5 core elements of the MTM service model
•List opportunities for providing MTM services 
•Discuss quality measures that can be impacted by MTM services
